The IRC protocol is a protocol which was designed when the
internet was a much friendlier place.
Over the years, and as the internet changed, the protocol was
changed to face the fact that you simply cannot trust clients
(at least on public networks).  However, this is a lot harder
to make the same changes when it comes to servers, and it
remains essential that server admins be trusted.

V lines are no more or less effective than many other things.
However, they are a tool which many admins have found
somewhat useful.
