logging of unseccessful /op

hi,

imho ircd should log each unseccessfull /op in FNAME_OPERLOG, providing
identd-reply and ip-address (since DNS might be spoofed to match O:-line)
of the client as well.  a note on failure to &local (maybe for op'ed clients
only) would be nice.

it's really strange some things concerning security are dealt with but
one of the few passworded things is not monitored at all.
